Supporting your health care journey
Our nurse navigators are experienced nurses who work with you, your family and your health care team. We help you understand the health system and stay as healthy as possible.
We can:
- explain your medical conditions and treatment
- work with your GP, specialists and health teams
- support you to look after your health at home and in your community
- connect you to services and support
- go to appointments with you if needed
- help with advance care planning
- speak up for your needs
- help you plan your care.
We have nurse navigators that focus on:
- respiratory care
- diabetes
- child development
- palliative care
- sexual health
- rheumatic heart disease
- patient travel support.
Nurse navigators will also support you if you live in areas outside of Mount Isa, such as Doomadgee, Mornington Island, Cloncurry, Camooweal and Normanton.
Who we help
We support people who have long term and complex health conditions. We help if it’s hard for you to look after your health or get the care you need.
We may be able to help if you:
- have more than one health condition
- see lots of different health providers
- find it hard to keep track of appointments
- live in a remote community
- need help to follow your care or treatment.

Your health care professional can also refer you. They'll ask for your consent first.
